Improving health and reducing inequalities in health


Strategies

The following strategies are in place to provide guidance and a framework for local developments:

  • The Neighbourhood Renewal Strategy

  • Draft Tobacco Control Plan

  • Healthy Eating/Nutrition: a number of strategies are available that provide guidance and information

  • Mental Health Promotion Strategy
    The Mental Health Service User/Carer Strategy

  • County Durham and Darlington Teenage Pregnancy Strategy

  • County Durham and Darlington Healthy School Standard: The County Durham
    Healthy School Standard was accredited against the national standard in 2001.

  • County Durham and Darlington Physical Activity Strategy


Partnerships

  • The Derwentside Partnership has a multi-agency health theme group, which is also the health improvement group for the PCT.

  • There are two Sure Start partnerships in Derwentside, Sure Start Stanley and Sure Start West Derwentside.

  • The Derwentside Healthy Living Centre Consortium.

  • Stanley Green Corridor Partnership. 


Improving access to and quality of health and social care services.


Strategies and partnerships

To date strategies for improving access to and quality of health and social care services have been developed in relative isolation. However a new Partnership Committee for health, social care and housing services for adults has recently been formed. This aims to ensure that all vulnerable adults and their carers receive the most effective and efficient support services that can be provided for them.

Patient and public involvement

Strategies

  • The NHS Plan and Section 11 of the Health and Social Care Act guide the development of patient and public involvement.


Partnerships

There are a number of key partnerships that encompass the involvement of patients, communities and the wider public:

  • Derwentside Health Improvement Group

  • Derwentside PCT Community Reference group

  • Derwentside Community Network

  • Local Community Partnerships

  • The Consultation & Communication sub-group of the Derwentside Partnership
