Key Issue 1:

Higher mortality rates for the major diseases
 

Aim:

Improving Health

 

Objectives:

  • Tobacco control including smoking cessation services.

  • Promoting better mental health, and preventing chronic diseases and accidents.

  • Provide improved access to physical activities opportunities which promote a healthier lifestyle.

Key Issue 2 - Inequalities in the levels of health between people living in Derwentside


Aim:

Reducing health inequalities

 

Objectives:

  • Provide a sure foundation through a healthy pregnancy and early life chances.

  • Strengthen disadvantaged communities and address the needs of vulnerable groups.

Key Issue 3:

Bring services to the patient, making best use of the
skills of the local workforce, new technologies and improvements to premises


Aim:

Improving Access to Health Services

 

Objectives:

  • To ensure that the population of Derwentside is able to access health services in line with national waiting time targets.

  • To ensure that primary care and community services are provided in easily accessible, modern facilities that are fit for purpose.

Key Issue 4:

Provision of care that is related to need, effective,
makes best use of available resources and minimises unacceptable variations in clinical practice

Aim:

Fully implement the National Service Frameworks

 

Objectives:

  • To ensure that National Service Frameworks (NSFs) are implemented consistently by providers of health care in Derwentside.

  • To work with partners across the county to develop a consistent approach to assessment (single assessment process) and implement the process in Derwentside.

Greater involvement of patients and the public in
individual treatment, planning services and addressing local needs

Aim:

Expand patient and public involvement

 

Objectives:

  • To ensure that individual patients and their carers are fully involved in their care and treatment.

  • To ensure that patient and public involvement is included at all levels of planning, delivery, monitoring and evaluation of health services.
