Bring services to the patient, making best use of the
skills of the local workforce, new technologies and improvements to premises

Improving Access to Health Services
Objectives:

To ensure that the population of Derwentside is able to access health
services in line with national waiting time targets.

To ensure that primary care and community services are provided in
easily accessible, modern facilities that are fit for purpose.

Provision of care that is related to need, effective,
makes best use of available resources and minimises unacceptable variations in
clinical practice
Aim:
Fully implement the National Service Frameworks
Objectives:

To ensure that National Service Frameworks (NSFs) are implemented
consistently by providers of health care in Derwentside.

To work with partners across the county to develop a consistent approach
to assessment (single assessment process) and implement the process in
Derwentside.

Greater involvement of patients and the public in
individual treatment, planning services and addressing local needs
Aim:
Expand patient and public involvement
Objectives:

To ensure that individual patients and their carers are fully involved
in their care and treatment.

To ensure that patient and public involvement is included at all levels
of planning, delivery, monitoring and evaluation of health services.
