Fully implement the National Service Frameworks.

| To ensure that National Service Frameworks (NSFs) are
implemented consistently by providers of health care in Derwentside. |
| Action |
- Continue to identify developments required to meet national
standards for all published NSFs and submit plans for prioritisation as
part of funding allocations.
- Support practices to collect good quality data to support audit and
evaluation.
- Develop and deliver multi-agency training programmes.
- Progress establishment of Partnership Committee to facilitate closer
working and delivery of integrated services.

implement the process in Derwentside. |

- Agree new integrated ways of working built on sharing of information
with partners in health and social care i.e. acute and mental health
providers, Social Services and District Council.
- Support development of electronic record that can be accessed from
many locations.
- Agree changes with patients and public.
- Adopt single assessment process as a way of determining need and
improving care planning.
