Aim
:

Improving Access to Health Services.


Objective:
To ensure that the population of Derwentside is able to access health services in line with national waiting time targets.
Action
  • Support the development of emergency care practitioners in the community (ambulance service).
  • Expand intermediate level services in the community.
  • Increase levels of services commissioned from specialist providers.
  • Expand the capacity of care services to support earlier discharge from hospital and avoid unnecessary admissions to hospital or residential care.
  • Expand use of local buffer stores of community equipment.
  • Promote access to health services via a single telephone call to NHS Direct.
  • Develop IM&T infrastructure to prepare for universal electronic booking of appointments.

Impact by 2010
  • Maximum four hour wait in A&E Departments.
  • Single telephone call access to NHS Direct for services out of hours.
  • Ability to see a GP within two working days of request.
  • Ability to see a primary care professional with one working day of request.
  • Maximum three month wait for hospital outpatient appointment by December 2005.
  • Maximum six month wait for inpatients by December 2005.
  • Increase level of choice of provider and option to book appointments at point of referral by December 2005.

Objective:
To ensure that primary care and community services are provided in easily accessible, modern facilities that are fit for purpose.
Action
  • Replace Stanley Health Centre with a new Primary Care Centre.
  • Review role of South Moor Hospital to ensure best use is made of facilities.
  • Progress improvement schemes with practices planning to re-locate or improve current accommodation to meet needs of registered populations.
  • Develop facilities at Shotley Bridge Community Hospital and new Stanley Primary Care Centre for the provision of intermediate level services.

Impact by 2010
  • Maximum one month wait from diagnosis to treatment for cancer and two months maximum wait from referral to treatment for urgent cancers by December 2005.
  • Modernised services need modern facilities that provide safe, pleasant surroundings that support new ways of working.
  • Contribute to a national target to replace or refurbish 3000 GP premises by the end of 2004.
  • Establish new one-stop primary care centres by 2006.
  • Primary care premises should be clean, comfortable, friendly places to be.
