- Support the development of emergency care practitioners in the
community (ambulance service).
- Expand intermediate level services in the community.
- Increase levels of services commissioned from specialist providers.
- Expand the capacity of care services to support earlier discharge
from hospital and avoid unnecessary admissions to hospital or
residential care.
- Expand use of local buffer stores of community equipment.
- Promote access to health services via a single telephone call to NHS
Direct.
- Develop IM&T infrastructure to prepare for universal electronic
booking of appointments.

improve current accommodation to meet needs of registered populations.
- Develop facilities at Shotley Bridge Community Hospital and new
Stanley Primary Care Centre for the provision of intermediate level
services.

two months maximum wait from referral to treatment for urgent cancers by
December 2005.
- Modernised services need modern facilities that provide safe,
pleasant surroundings that support new ways of working.
- Contribute to a national target to replace or refurbish 3000 GP
premises by the end of 2004.
- Establish new one-stop primary care centres by 2006.
- Primary care premises should be clean, comfortable, friendly places
to be.
